An imaging finding is not a sentence
Partial cuff tears and tendon changes show up on scans of plenty of pain-free shoulders. What matters is whether the shoulder can produce force and move well — both of which respond to training.
The phases
- Settle the irritation. Modify aggravating positions, manual therapy, isometric holds for pain relief.
- Rebuild capacity. Progressive rotator cuff and scapular strengthening, three days a week.
- Restore overhead function. Loading in the positions your job or sport demands.
Expect 8 to 12 weeks
Tendon adaptation is slow. Programs abandoned at week four are the reason so many shoulders feel like they never fully resolved.
