Appointment requests made through the website are not confirmed until our staff contacts you. Insurance plans listed on the site are those we commonly work with; coverage depends on your individual plan, and we recommend calling or texting us to verify benefits before your visit. You are responsible for any co-pays, deductibles and services not covered by your plan. Please give us at least 24 hours' notice if you need to cancel or reschedule.
