Nine months is the number athletes hear. But ACL rehab in NJ should be driven by criteria, not by a date on a calendar.
Phase one: quiet knee
Full extension, minimal swelling and a quad that fires properly. Extension lost early is very hard to regain later.
Phase two: strength
Progressive quad, hamstring and glute strengthening. Blood flow restriction training is useful here when heavy loading is not yet tolerated.
Phase three: testing
- Quad strength within 10% of the other leg
- Hamstring-to-quad ratio appropriate for the sport
- Hop test battery within 10% symmetry
- Controlled landing and cutting mechanics on video
- Confidence questionnaires — fear of reinjury predicts outcomes
Why this matters
Returning before strength symmetry is restored is consistently associated with higher reinjury rates. Passing criteria is worth the extra few weeks.
Then keep training
Strength and plyometric work continues through the first season back, not just until clearance.
